Representation Agreement
The terms of this Agreement, by and between _________________, hereinafter referred to as "Client" and The Law Office of Kevin Kennedy, hereinafter referred to as "Law Firm" are set forth as follows:
1. Client hereby retains and employs Law Firm to represent him/her for his/her traffic citation received on ______________, 20__ for __________________________________________.
2. The attorney's fees will be as follows: $______.00 for representation on the above-mentioned citation only.
3. Client understands that the above fee is a flat fee and that no part thereof shall be returned to the client. Further, this fee includes all fines to be assessed by the court.
4. Client understands that Law Firm will not dispose of any case until the fee is paid in full. Law Firm is herein given the right to continue this case at its discretion up to three (3) weeks from the original court date listed on the citation to facilitate receipt of the signed Waiver of Appearance and this contract. Client further understands that another law firm may be associated to resolve this case.
5. Client understands and agrees that if they do any of the following: a) fail to pay the fee in full; b) fail to return a signed Waiver of Appearance or; c) fail to return the signed Contract of Representation within three (3) weeks of the original court date as listed on the citation, Law Firm may withdraw from representation without notice and may retain that portion of the fee previously paid as compensation for its services rendered in continuing said cases.
6. Client represents that he/she has revealed all traffic violations received in ANY STATE within the last three (3) years.
7. Client also agrees that Law Firm shall not be required, without further agreement, to pursue an appeal in the event one becomes necessary after a hearing or trial.
8. Client understands that another firm may be associated to resolve his/her case as need be determined in the discretion of Law Firm. A portion of the fee will be tendered for this purpose.
Client now reveals all traffic violations received in the past three (3) years and further acknowledges an ongoing obligation to update this information should he/she receive any additional citations or convictions:
